- For You: $5,000, $10,000, $20,000, or $30,000 cash benefit levels
- For Your Spouse: $5,000, $10,000, $20,000, or $30,000, up to the amount you elect for your coverage
- For Your Children: $5,000, $10,000, or $15,000, up to the amount you elect for your coverage (at the same cost regardless of number of children)
