Statewide access to a network of panel providers.
No claims to file for "in-network" benefits.
Benefits for out-of-network providers.
The Anthem Vision Care participating provider network includes private practice optometrists, ophthalmologists and retail chains.
What You Should Know
The Select Plan benefits include but are not limited to:
Eye exam and eyeglass lenses every year, copayments apply.
$130 allowance for frames every two years.
Prescription contact lenses allowance of $105. To receive the full allowance, you must receive your exam, fitting and evaluation at a single visit to the same network provider.
Any remaining balance not used during the plan year when the purchase occurred will be forfeited.
There are network and out-of-network benefits.
